Background
Obesity is a global chronic health condition. Whether through calorie restriction or pharmacological weight-loss interventions, weight reduction may be accompanied by the loss of skeletal muscle mass. Some studies have suggested that with potent weight-loss medications, muscle may account for a substantial proportion of total weight loss, raising concerns about the potential risks of sarcopenia and declining physical function.
Calcium β-Hydroxy-β-Methylbutyrate (HMB-Ca) is the calcium salt of HMB, an active metabolite of the essential amino acid leucine. Previous research in sarcopenia and sports nutrition has highlighted its potential muscle-preserving benefits.
However, whether HMB-Ca can help achieve the goal of “fat loss while preserving muscle” during weight-loss interventions remains a question that requires validation through higher-quality human clinical evidence.
Published in the Journal of Cachexia, Sarcopenia and Muscle, this study was a randomized, double-blind, placebo-controlled, parallel-group clinical trial conducted by West China Hospital.
A total of 102 Chinese adults with obesity, aged 30–50 years with a BMI ≥28 kg/m², were enrolled and randomly assigned in a 1:1 ratio to either the HMB-Ca group or the control group. The intervention lasted 12 weeks, with all participants following a standardized energy-restricted diet during the study period.
Participants in the HMB-Ca group consumed 65 g of a solid beverage daily, providing 3.0 g of HMB-Ca and 24 g of protein, while the control group received an energy-matched maltodextrin formulation.
The primary endpoint was the change in total skeletal muscle mass, which was assessed using bioelectrical impedance analysis (BIA). This rigorous clinical design provided a strong basis for evaluating the potential role of HMB-Ca in preserving skeletal muscle during weight loss.
No adverse events were reported throughout the study, and no significant between-group differences were observed in safety parameters, including liver and kidney function, indicating that HMB-Ca was generally well tolerated during the 12-week intervention.
However, several limitations should be considered when interpreting the findings. First, the intervention formula contained 24 g of protein, making it difficult to completely isolate the independent effects of HMB-Ca from those of protein supplementation. Second, skeletal muscle mass was assessed using bioelectrical impedance analysis (BIA) rather than more precise imaging techniques. In addition, the intervention period was limited to 12 weeks, and secondary and subgroup analyses were not subjected to multiple-comparison correction.
Therefore, while the results provide encouraging clinical evidence for the potential of HMB-Ca to support muscle preservation during weight loss, further studies with longer intervention periods and more accurate assessment methods, such as DXA or MRI, are needed to confirm its long-term efficacy and independent effects.
This randomized, double-blind, placebo-controlled clinical trial provides valuable human evidence on the potential of HMB-Ca supplementation to improve the quality of weight loss in adults with obesity undergoing energy restriction.
Key Findings:
During the 12-week weight-loss intervention, the HMB-Ca group maintained skeletal muscle mass, with a net between-group benefit of 1.3 kg, while also achieving greater reductions in visceral fat and total body fat. In addition, basal metabolic rate was better maintained, suggesting a potential nutritional strategy for supporting fat loss while preserving lean mass.
Clinical Significance:
The findings highlight that successful weight management should go beyond simply reducing body weight and should also consider body composition and muscle preservation. HMB-Ca may offer a nutritional approach to help address skeletal muscle loss associated with calorie restriction, with potential relevance to individuals undergoing weight-loss interventions, including those at risk of sarcopenic obesity.
Mechanisms & Evidence Boundaries:
The findings are consistent with proposed mechanisms whereby HMB may support muscle protein synthesis through the mTORC1 pathway, reduce ubiquitin–proteasome-mediated protein degradation, and help maintain muscle cell membrane integrity. However, because the intervention formula also provided 24 g of protein, the independent contribution of HMB-Ca cannot be fully separated. The relatively short 12-week intervention period also limits conclusions regarding long-term benefits. Further factor-controlled studies with longer follow-up are needed to confirm the independent and sustained effects of HMB-Ca.
The muscle-preserving benefits of HMB-Ca (Calcium β-Hydroxy-β-Methylbutyrate) have been extensively investigated in the fields of sarcopenia and sports nutrition. Its calcium salt form further expands its potential applications across weight management, sports nutrition, and healthy aging.
